Pharmacy Patient Assistance Program

Pharmacy patient assistance programs can help families and St. Jude.

Drug companies have programs that give free or less expensive medicine to people who need help paying for their medicines.

These patient assistance programs also help St. Jude Children’s Research Hospital. Because St. Jude does not pay for these medicines, we can use the savings to pay for other hospital needs.

Services we provide

Health insurance may not pay for a prescription medicine. Or it might pay for only part of it.

You might qualify for pharmacy patient assistance programs if:

  • You are a U.S. resident or citizen
  • Your annual income is at or below a certain amount, usually 200% of the United States federal poverty guidelines
  • You do not have prescription drug insurance that covers your child’s medicines

A pharmacy patient assistance specialist will help you apply for patient assistance programs.

Each drug company has its own requirements. If you qualify, your child will receive free or discounted medicines.

Details we need from you

The specialist needs the following information:

  • Proof of income
  • IRS Form 4506, Request for Copy of Tax Return
  • IRS Form W-2, Wage and Tax Statement
  • Personal information such as date of birth, Social Security number, and household size
  • Patient or parent signatures
  • A doctor’s signature (The patient assistance specialist will get this for you.)

The doctor sends the prescription to the pharmacy. Then, the pharmacy sends the request to the company. If you qualify for the program, the company sends your medicine to St. Jude. The pharmacy staff fills the prescription for you.

If your insurance changes or you get a new job, please tell the patient assistance specialist right away.
