The Cancer Predisposition Transition of Care Program helps teens and young adults with cancer predisposition conditions make a smooth transition from pediatric to adult care providers. Our approach is to protect privacy, help you learn to advocate for yourself, and foster trust with future health care providers.
When you are a child or teen, your family caregivers make most of your health care decisions. That changes when you turn 18 and become a legal adult. After 18, most young adults in our program move to an adult health care provider.
Planning and preparing to move from pediatric to adult care begins at least 2–4 years before you move to an adult health care provider.
During your visits, we will:
One of our genetics providers will meet with you and your legal guardian before you begin seeing adult care providers. We will review your cancer predisposition condition and discuss health risks. This review can inform decisions about having children in the future. We will also work with you to find community and financial resources.
Some young adults have conditions that limit them from making health care decisions on their own. In these cases, our team provides family caregivers with options for how to support their child’s decision-making. Young adults who are unable to consent for their own health care or make their own decisions will need a legal document that describes their decision-making limits. This document also makes clear who can give consent on their behalf.
