How to Refer Your Patient

Requirements for acceptance to an open St. Jude clinical trial include age range, type and stage of disease, and other medical criteria.

For most diseases and treatments, St. Jude accepts individuals age 21 or younger at the time of acceptance. Exceptions to this age limit include:

• Those age 18 or younger with:
  • Acute lymphoblastic leukemia (ALL) and some other forms of childhood leukemia
  • Non-cancerous blood disorders
    
  • Genetic predisposition for cancer (passed down in families)
    
• Those age 25 or younger with:
  • Certain solid tumors
  • Certain brain tumors
    
  • Surgery
    
  • Radiation oncology

When you contact St. Jude, please have the following details ready:

• Name and contact information of the care provider making the referral
• Patient’s name
• Patient’s date of birth
• Patient’s address and phone number
• Patient diagnosis and treatment history

• You can also submit referral requests through St. Jude Care Everywhere (if you are part of the Epic or Carequality networks) or through St. Jude EpicCare Link if you work in a hospital or group that does not have the Epic EHR system.
• For specific service lines, you can also reach out to individual referral contacts. These include: 
  • Brain Tumor Program
    
  • Department of Surgery
• To refer a child to a St. Jude affiliate clinic, call the specific affiliate.

In some cases, St. Jude may need more specific information as part of a referral. This could include:

• Clinical summary from diagnosis to present, including all specific treatment flow sheets, dates, and doses of chemotherapy and/or radiation therapy
• All appropriate diagnostic material including paraffin blocks, H & E slides of the tumor, or bone marrow aspirate and biopsy slides
• All CT, MRI or nuclear medicine films and reports from the date of diagnosis
• The most recent disease evaluation. This can include either imaging studies or marrow aspirates and biopsies
• Surgery dates and OR reports
• Immunocytology or immunopathology reports, including all flow studies or molecular studies
• Any cryopreserved tumor, stem cells or marrow reports
• The most recent general evaluation, including ECHO, EKG, cardiac, liver function tests and metabolic studies
• Any other appropriate information specific to the tumor type

The timeline for patient referral and acceptance may vary depending on whether a trial is open, the time it takes to gather records and ship samples, the time to process the request, clinic appointments, and other factors.
