This form is designed to provide students, employees, faculty, vendors, visitors, or others with an on-line method to report specific information related to an alleged incident(s) of sex and gender harassment and discrimination policies, and sexual misconduct within an educational program or activity. In all cases the reporting party information will be shared with as few people as possible and every effort will be made to maintain privacy.
We encourage you to provide as much detail as possible as to what occurred either now or later during your meeting with the Title IX Coordinator. If you are not the Complainant, you may provide the names of the Complainant, the Respondent, and witnesses, to enable a more complete response. If the Complainant is named, the Title IX Coordinator will contact the Complainant to offer supportive measures, and to ask if the Complainant would like to pursue an investigation.
